Providing an active day facility for seniors and disabled adults

Daily Grace Adult Day Health Services provides disabled adults and seniors that are mentally and/or physically disabled with the ability to have an active life through social, medical, and physical support. We offer their caregivers peace of mind by creating a safe, comfortable and supportive environment where their loved ones can thrive.

Daily Grace Adult Health Services works with each family and care receiver to determine the best level of care needed. Our program provides safe group environments with coordinated health and social services aimed at stabilizing or improving self-care.

We serve residents of Bibb, Jones, Monroe, Twiggs, Wilkinson, Baldwin, Houston, Peach, and Crawford counties.

Services

We provide Adult Day Health services from 8:00am to 2:00pm, Monday thru Friday by appointment.  Our program includes

 

  • Nutritious Meals (ServeSafe Certified Staff Member)

  • Skilled Therapy 

  • Skilled Nursing Services

  • Social Interaction With Their Peers

  • Therapeutic Activities

  • Group Exercise Programs

  • Arts and crafts

  • Cognitive games and activities

Appropriate clients for this program

 

Daily Grace Adult Health Services recognizes that our service strategy is best directed to customers with a specified range of service needs. Participants who are appropriate may have a degree of physical disability, social impairment, mental confusion, and/or customers who may need assistance with activities of daily living which fall short of the need for placement in a skilled nursing facility or memory care center. Clients who have returned from a recent hospital or institutional stay may also be appropriate for services when they present needs as outlined above. Persons who would like to continue their rehabilitation care can do so here and return home after their daily care. We accept individuals with Dementia as well in the early to middle stages. We offer the Skills2Care program for caregivers of persons with Dementia.
