Costs for Services

Daily Grace Adult Day Health Services is committed to providing high-quality, person-centered care that meets the unique needs of each participant. Our goal is to exceed expectations through compassionate support, meaningful engagement, and professional clinical oversight.

The cost of services is based on the individual’s level of care and the service package selected. Each participant receives an initial assessment conducted by a Registered Nurse to determine the appropriate level of care, supervision, and support needed to ensure safety, health, and overall well-being.

If there is a change in the participant’s care needs that affects the level of care or service package, the participant or their authorized representative will be notified both verbally and in writing. Notification of any fee adjustments or changes in benefits will be provided at least seven (7) business days in advance of the change whenever possible.

We strive to provide transparent pricing and will work with families, caregivers, case managers, and funding sources to identify appropriate payment options.

Accepted Payment Sources

Daily Grace Adult Day Health Services may accept the following payment sources, depending on eligibility and program requirements:

• Private Pay (self-pay)
• Private Insurance (coverage varies by plan)
• Long-Term Care Insurance Policies

Medicare & Dementia Support Programs

• CMS GUIDE Model (Guiding an Improved Dementia Experience) – dementia care coordination and caregiver support services available to eligible Medicare beneficiaries

Participants with Traditional Medicare Part B, living with dementia and their caregivers may qualify for additional support services through the CMS GUIDE Model, which focuses on care coordination, caregiver education, and resources to support aging in place.

Medicaid Waiver Programs

• Elderly and Disabled Waiver Programs – SOURCE and CCSP
• Department of Behavioral Health and Developmental Disabilities (DBHDD) Waivers – NOW and COMP

Veterans Benefits

• Veterans Affairs (VA) Geriatrics and Extended Care Services (Form 10-10EC)
• Aid and Attendance Benefit
• VA Healthcare System Enrollee Benefits
• VA Community Care Network

Additional Support Options
